Tardy Log - you must complete this form anytime you arrive in class after the tardy bell.  Excused or Unexcused!

Hall Pass - you must complete this form when you leave the classroom anytime for any reason before the dismissal bell.  


Tutorial Log -  you must complete this form when you attend a tutorial.

Late / Make Up Work Form - you must complete this form anytime you turn in an assignment after the due date.  Excused or Unexcused.  This is the only way to receive credit for this work.

Textbook Check Out / Check In Form - you must fill out this form to check out a Textbook.  Be sure you check it back in when your return it to school.
